Alumni Chapters - Event Supplies Request Form

Please note the following:

  1. Event supply requests must be submitted a minimum of two weeks prior to the date the supplies are needed.
  2. Quantities requested cannot exceed the maximum amount listed next to each specific item unless your event qualifies as a major event.
  3. Supplies must be requested for individual events. We cannot accept bulk requests that exceed the maximum amount for each respective item. If you would like to request supplies for multiple events due to a large amount of consecutive events, please email rather than filling out this form.
